NHS Ayrshire and Arran: Pay and Health Board Structure
Last updated September 2026 · Independent guidance, not affiliated with NHS England or DHSC
NHS Ayrshire and Arran is one of Scotland's territorial Health Boards, covering mainland Ayrshire together with the island of Arran — a mix of urban, rural and island geography that's built into the board's name itself. Like the rest of NHS Scotland, it plans and delivers hospital and community services directly for its area rather than sitting alongside a separate network of trusts. If you're used to England's trust-and-ICB structure, our explainer on how the NHS is structured sets out how the four nations' models actually compare.
Pay: Scotland's own scale, not England's
This is the point worth getting right before anything else: NHS Ayrshire and Arran does not use the same pay table as England, Wales or Northern Ireland. NHS Scotland restructured its Agenda for Change pay points in 2018 and has negotiated its own separate annual pay award ever since, via the Scottish Terms and Conditions Committee rather than the England/Wales/NI process. The band structure — Band 2 through Band 9 — is recognisably the same UK-wide, but the actual point-by-point figures differ. Our NHS Scotland pay scales guide has the current rates in full, and the NHS Pay Calculator will apply them automatically if you select Scotland.
No location weighting, unlike London
England's High Cost Area Supplement exists specifically because of London's cost of living, and it has no equivalent anywhere in Scotland. Whether you're based on the Ayrshire coast or on Arran itself, pay at NHS Ayrshire and Arran is the standard Scotland Agenda for Change rate for your band and point — there's no separate zone calculation to factor in the way there would be at a London trust.
A mixed mainland-and-island health board
Serving both a mainland population and an island community gives NHS Ayrshire and Arran a genuinely broad service footprint, and with it the typical spread of NHS roles you'd expect from a health board of this kind — nursing, medicine, allied health professions, healthcare support workers, and administrative and estates staff, all employed on the Scotland Agenda for Change scales. Our nursing and allied health professionals guides cover how pay progresses band by band, which applies here exactly as it does at any other Scottish health board.
Your employer within NHS Scotland
NHS Ayrshire and Arran, not "NHS Scotland" generically, is your specific employer if you work here, which is what matters for continuous service calculations. See our redundancy pay guide for the general principles, bearing in mind that Scotland's own procedures can differ in detail from England's.
Frequently asked questions
Is NHS Ayrshire and Arran a trust or a health board? +
It's a Health Board — Scotland doesn't use the NHS trust model that England does. NHS Ayrshire and Arran plans and delivers NHS services directly across its area, covering mainland Ayrshire and the island of Arran, rather than sitting alongside separately managed trusts locally.
Does NHS Ayrshire and Arran use the same pay scale as an English trust? +
No — Scotland restructured its Agenda for Change pay points in 2018 and has run its own separate scale ever since, with its own annual pay award agreed by the Scottish Terms and Conditions Committee. The band numbers (2 through 9) match the rest of the UK, but the exact figures for each point are Scotland-specific — see our Scotland pay scales guide for the current rates.
Is there a High Cost Area Supplement anywhere in Scotland? +
No — HCAS is an England-specific arrangement tied to London and its fringe. NHS Scotland, including NHS Ayrshire and Arran, doesn't operate an equivalent zoned supplement, so pay is simply the Scotland Agenda for Change rate for your band and point.
