NHS Band 8b Salary 2026/27: Full Pay & Who's on This Band
Last updated September 2026 · Independent guidance, not affiliated with NHS England or DHSC
Band 8b is where the job stops being "running a team" and starts being "running a service." At Band 8a, most people still have a direct line to patients, or a single team whose performance is theirs to own. At Band 8b, you're typically accountable for a whole department — several teams, several Band 8a leads reporting to you, and a budget that belongs to you rather than one you simply work within. It's a genuine step up in scope, and it's worth being honest about something rarely said out loud in NHS careers content: these posts are scarcer than the ones below them. This page covers the real 2026/27 figures, then what that scarcity actually means for anyone aiming at 8b.
Short answer
NHS Band 8b pays between £66,582 and £77,368 a year in England, Wales and Northern Ireland for 2026/27, across two pay points. Scotland's Band 8b runs from £76,888 to £82,251. Fewer Band 8b posts exist per trust than Band 8a posts, so progression here is a genuine bottleneck for many senior clinicians and managers.
Band 8b pay points 2026/27 (England, Wales & Northern Ireland)
Heads of department, senior clinical leads. For context, Band 8a tops out at £64,751 — here's the full Band 8b scale it steps up to:
| Pay point | Annual salary (2026/27) |
|---|---|
| Point 1 — starting salary on promotion | £66,582 |
| Point 2 — top of Band 8b | £77,368 |
In Inner London, High Cost Area Supplement adds roughly £8,746 on top of the £77,368 top-of-band figure — a meaningful sum at this salary level. See the full Agenda for Change pay scales guide for the complete HCAS table.

Who holds a Band 8b post?
- Nurses (Band 5–8c) — Adult, mental health, children's and learning disability nursing across every NHS setting.
- Allied Health Professionals (Band 5–8b) — Physiotherapists, occupational therapists, radiographers, speech & language therapists, dietitians and podiatrists.
- Healthcare Scientists (Band 5–8c) — Lab-based and physiological science roles spanning pathology, physics, physiology and genomics.
- Pharmacists (Band 4 (technician) – Band 8c) — Hospital pharmacists and pharmacy technicians, including independent prescribers.
- Admin & Estates Staff (Band 2–8d) — The non-clinical backbone of the NHS: administrators, managers, porters, cleaners and estates staff.
In nursing and allied health, typical 8b titles are head of nursing for a division, deputy head of therapies, or a senior consultant-level clinical post covering a whole specialty rather than a single team's caseload. In pharmacy, it's usually a chief pharmacist or principal pharmacist covering a large directorate. On the general-management side, 8b is where a genuinely broad head-of-department remit — finance, estates, workforce, digital — first appears, entirely separate from any clinical qualification. See our admin, estates and general management careers guide for how that non-clinical route reaches this level.
Why Band 8b is a real bottleneck, not just a bigger title
NHS organisations are structured like a pyramid, and it gets noticeably narrower above 8a. A large acute trust might have dozens of Band 8a matron, service-lead and consultant-nurse posts, but only a handful of Band 8b head-of-department roles sitting above them — because a head of department typically has several 8a leads reporting in, not the other way round. That arithmetic means fewer vacancies open up in any given year, and the ones that do tend to attract strong internal and external competition. None of this makes 8b unreachable — people move into it every year — but it does mean the realistic path usually involves patience, a track record built up over several years at 8a, and a genuine willingness to look beyond your current trust if the right post isn't coming up locally.
ℹFewer posts means the search often has to widen
If you're aiming for Band 8b and nothing suitable has opened up at your own trust in a while, that's not necessarily a sign you're not ready — it may just reflect how few of these posts exist at any one organisation. Widening your search regionally, and being open to a head-of-department post in a smaller or less familiar trust, is one of the most practical ways people actually get there, rather than waiting for an identical role to become available where they already work.

Where next after Band 8b?
Band 8c is the next step, and it's where NHS pay genuinely starts to compete with senior private-sector leadership salaries — deputy directors and senior consultant-level clinicians for 2026/27 start at £79,505. How many years does it typically take to reach Band 8b? An honest answer
Given how few Band 8b posts exist relative to Band 8a ones, this is the band on the whole ladder where a "typical years" figure would be the least honest thing we could publish. The limiting factor usually isn't how long someone has spent preparing — it's how often a suitable vacancy genuinely appears, and that varies enormously by specialty, region and even by which trust you happen to be at when one opens.
Some people move from 8a into 8b within a few years of taking up their first senior post, simply because a head-of-department role happened to open in their exact specialism at a trust willing to consider them. Others spend considerably longer at Band 8a — not because they aren't ready, but because nothing suitable has come up, and widening a job search regionally, or being open to a less familiar trust, is often what eventually closes that gap rather than the simple passage of time.
It's worth being explicit about what this means practically: staying at Band 8a for an extended period is not, on its own, evidence that someone has stalled. Given how the organisational pyramid narrows above 8a, a longer wait is the structurally normal experience for most people aiming at this level, not the exception.

Frequently asked questions
What does NHS Band 8b pay in 2026/27? +
Band 8b runs from £66,582 at point 1 to £77,368 at the top of the band in England, Wales and Northern Ireland for 2026/27. NHS Scotland's Band 8b runs from £76,888 to £82,251.
Is Band 8b actually harder to get than Band 8a? +
In relative terms, usually yes. A typical trust employs meaningfully fewer head-of-department and senior-clinical-lead posts than it does matron, service-lead and consultant-nurse posts at 8a — the organisational pyramid narrows as you go up. That doesn't mean 8b posts never come up, but it does mean the honest expectation should be more applications, more competition per vacancy, and often a wider geographic search before you land one, compared with the jump from 8a into 7.
What's actually different about the job at Band 8b compared with Band 8a? +
At Band 8a you're typically still running a single ward, team or specialist caseload, even if you also carry some wider responsibility. At Band 8b you're usually accountable for an entire department or service line — several teams, several Band 8a leads reporting into you, a real budget you own rather than contribute to, and much more exposure to trust-wide strategy, risk and governance meetings. It's a genuine step change in scope, not just a bigger version of the same job.
Do I need to move trusts to get promoted to Band 8b? +
Not necessarily, but it's common. Because 8b posts are relatively scarce within any single organisation, waiting for one to open up internally can mean a long wait. Many people who reach 8b do so by applying more broadly across their region — sometimes moving trusts, sometimes moving from a large teaching hospital into a smaller trust or an integrated care board where a head-of-department post becomes available sooner.
Does an MBA or equivalent management qualification actually help at this level? +
It's increasingly common rather than strictly required. Some 8b and above posts, particularly on the general-management side, list a postgraduate management qualification (an MBA, MSc in Healthcare Management, or similar) as desirable. On the clinical-leadership side, it matters less than demonstrable service-improvement, governance and financial-management experience — but either way, it's rarely the single deciding factor on its own; a track record of running something at scale carries more weight in most panels.
What does the Band 8b recruitment and interview process typically look like? +
Expect a longer and more formal process than at Band 8a, reflecting both the seniority and the scarcity of these posts. It commonly runs across multiple stages — a values-based and competency panel interview, followed by a presentation or case-study task to a wider panel that can include staff-side representation and colleagues from outside your immediate department. Because 8b posts are relatively rare within any one trust, don't be surprised if the process takes longer to schedule than a Band 8a recruitment simply because more senior panel members' diaries are harder to align.
Are Band 8b posts ever filled through recruitment agencies or interim arrangements rather than a standard NHS Jobs advert? +
Occasionally, yes — particularly for genuinely hard-to-fill specialist head-of-department posts, or where a trust needs interim cover while it runs a full permanent recruitment process. The great majority of substantive Band 8b posts are still advertised through the standard NHS Jobs or trust recruitment system in the normal way, so it's worth checking there first rather than assuming an agency route is the norm.
Does Band 8b pay differ between the general-management and senior clinical tracks? +
No — both sit on the same Band 8b pay scale for a given pay point, exactly as they do at Band 8a. The band reflects the level the post has been evaluated at, not which of the two career directions — head of department or senior consultant-level clinician — it represents.
How much does HCAS add to a Band 8b salary in London? +
In Inner London, High Cost Area Supplement adds roughly £8,746 to the £77,368 top-of-band Band 8b figure for 2026/27 — the same percentage-based calculation, subject to the same cash cap, that applies at every other band covered on this site.
