NHS Pay vs Working Abroad Calculator

Compare your real NHS take-home pay and pension with an actual offer you have, including the exchange rate, the cost of moving and the pay an offer has to reach to beat your NHS package.

This tool will not tell you what a role pays abroad, because nothing can reliably. It compares your NHS position with real numbers you already have. Why we build it this way.

Your NHS position

Where do you work?

Only used for the reference annual leave line.

The offer or research you are comparing it to

Every figure below comes from you: a real job offer or your own research from a primary source. FrontlinePay does not publish, look up or default any figure for what a role pays abroad. Why we build it this way.

Mid-market rate as of 7 Oct 2026 from ExchangeRate-API open data (open.er-api.com), daily mid-market rates. Exchange rates move daily, so check today's rate before relying on this. Check today's rate on XE

How do you know your pay abroad?

Best when you have it: a real net figure from an offer or payslip means FrontlinePay does not have to estimate any foreign tax.

Real extra pay from your own offer, comparable to why the NHS side counts pension value.

Insurance premiums, co-pays or excess your employer does not cover: the cost NHS free-at-point-of-use access does not ask you to budget for.

The cost of making the move (optional)

Visa and registration fees, exams, flights, shipping, deposits and a few weeks of temporary housing. Get these from the regulator, immigration site and quotes you have collected.

The one-off costs are spread over this many years.

What the offer has to beat

To match your NHS package, the offer abroad would need to pay about A$74,003 a year net.

That counts your NHS take-home and pension, your healthcare cost and employer retirement contribution abroad, converted at 1.9004.

1. Your NHS package, built up

Net take-home pay£26,129
+ One year's NHS Pension accrual, capitalised£12,812
= NHS package value£38,941

The pension figure is the 2015 CARE pension earned in a year multiplied by 20, the factor used across our pension tools. It is not a lump sum you can withdraw now. Leaving does not wipe out what you have built: with at least two years' membership it is normally kept as a deferred pension (see deferred benefits explained), and transferring it abroad is harder than people expect.

2. The offer abroad, built up (AUD)

Net payA$0
- Out-of-pocket healthcare cost- A$0
+ Employer retirement contribution+ A$0
= Package valueA$0
Converted to GBP at 1.9004£0.00

Exchange rate as of 7 Oct 2026. Check today's rate before relying on the conversion.

3. Annual leave: reference only, not added on top

The offer abroad

Not entered

NHS days include bank holidays. Leave is already inside the salary on both sides, so it is not added to either package.

What this comparison does not capture

  • It is illustrative, built from the figures you supplied. It is not a guarantee of either position.
  • Move costs are only as good as the figures you enter. Visa, registration and exam fees change, so check each regulator and immigration site.
  • It does not value free-at-point-of-use NHS healthcare beyond the cash healthcare cost you entered.
  • The abroad tax figure is only as accurate as the net pay or tax rate you typed in.

Short answer

The calculator works out your NHS package, net take-home plus one year of NHS pension, and holds it against the offer you enter: net or gross pay, employer retirement contribution, healthcare cost, exchange rate and move costs. It then tells you the net pay an offer would need to reach to break even. It contains no table of what any role pays in any country; every abroad figure is yours. See NHS pay compared to working abroad for why that is deliberate.

⚠What this cannot tell you

It is built only from figures you supply. It does not verify an offer, calculate foreign tax, or value free-at-point-of-use NHS healthcare beyond the cash cost you enter. Visa, registration and exam fees change, so check the official source for your destination before you rely on any move cost.

How to compare an NHS job with an offer abroad

Start from the offer, not from a salary table. Get the net annual pay in writing if you can, or the gross pay and a tax rate you have checked with the destination's tax authority. Then add what changes hands outside the headline pay: any employer retirement contribution, the healthcare you would have to pay for, and the one-off cost of getting there. Convert the total to pounds at a rate you trust, and compare it with your NHS package built the same way. The calculator does each of those steps and shows the working, so you can see which line is doing the work.

What is counted on each side

  • ✓ NHS take-home pay: your band and point (or a custom salary), with income tax, National Insurance, NHS Pension contribution and student loan from the same engine as the NHS Pay Calculator and the 2026/27 pay scales.
  • ✓ NHS pension value: one scheme year of 2015 CARE pension (1/54 of pay), valued at 20 times the annual pension earned, the convention used in our pension tools. See the 2015 Scheme guide.
  • ✓ Abroad net pay: supplied by you, as a net figure or as gross pay with a verified effective tax rate. We never calculate or default foreign tax.
  • ✓ Employer retirement contribution and healthcare cost: optional, from your own offer or research.
  • ✓ Move costs: optional one-off GBP costs spread over the years you expect to stay, with a payback figure if the offer pays more.
  • ✓ Exchange rate: the one number we supply, dated and editable.

Worked example: how to read the result

Suppose a Band 5, point 2 nurse in England in the NHS Pension Scheme has an offer with a net A$75,000 a year, an employer retirement contribution of A$8,000, A$2,000 a year of healthcare cost and £6,000 of one-off move costs over three years. These are invented numbers to show the method, not a typical salary for any role or country. At 1.9004 Australian dollars to the pound (7 Oct 2026):

NHS packageOffer abroad
Net pay£26,129A$75,000
Pension or retirement contribution+£12,812+A$8,000
Healthcare costnone entered-A$2,000
Package in pounds£38,941£42,623
Move costs per year (over 3 years)none-£2,000
After move costs£38,941£40,623

For these figures the offer would need to reach about A$71,804 net to match the NHS package once healthcare, retirement contribution and move costs are counted. The point of the break-even figure is that you can hold any other offer against it without redoing the sums.

Exchange rates and why you can edit them

Currency moves can swing a comparison by more than a pay rise, so we show the rate we used, the date and the source, and let you replace it. Rates are mid-market figures for 7 Oct 2026 from ExchangeRate-API's open data, and the tool links to XE so you can check today's. Your bank's rate on the day you transfer money will be less favourable than a mid-market rate, so treat the conversion as a best case.

The move costs most people forget

A one-off cost of a few thousand pounds is easy to overlook when you are comparing yearly pay, and it matters most if you only plan to stay a few years. The usual items are visa and registration fees, assessment or exam fees, flights, shipping, deposits and a few weeks of temporary housing. Add them from official fee pages and real quotes. If you might come back, also think about what leaving costs you here: a deferred pension you stop adding to, and any pay progression you would forgo.

What happens to your NHS pension if you go

With at least two years' qualifying membership your pension is normally kept as a deferred benefit and revalued until you draw it; with less, you are generally entitled to a refund of your own contributions. Our deferred benefits guide explains the rules and the two-year test. Moving the pension to an overseas scheme is a separate and riskier question, covered in transferring your NHS pension abroad. If you are weighing leaving the NHS for the private sector in the UK instead, see leaving the NHS for a private-sector job.

The healthcare cost line

As a UK resident you have access to NHS care free at the point of use. In several popular destinations, healthcare for residents, including healthcare workers and their families, can involve insurance premiums, co-payments or excess that do not exist here in the same way. The calculator subtracts the annual out-of-pocket figure you enter from the abroad side so it is not silently missing from the comparison. If you do not know it yet, finding out is more important than anything else on this page.

Sources and how we calculate

  • NHS side: the same 2026/27 pay scales, tax, National Insurance, NHS Pension tiers and 2015 CARE accrual used across FrontlinePay. See our editorial policy for how we source and update figures.
  • Exchange rates: ExchangeRate-API open data (daily mid-market), checked 7 Oct 2026; AED and SAR rates follow the pegs to the US dollar.
  • Everything else on the abroad side comes from you. Check tax with the destination's tax authority, fees with its regulator and immigration site, and costs with Numbeo-style sites of your own choosing.
  • Last updated October 2026.

Why this comparison holds up

  • ✓ No fabricated or looked-up figure for what any role pays in any country, consistent with our published guide.
  • ✓ The NHS side reuses FrontlinePay's verified pay, tax and pension engines rather than new mechanics for this tool.
  • ✓ The only figures we supply are dated, sourced exchange rates, and every rate is editable.
  • ✓ No default foreign tax rate anywhere: use your real net figure or a rate you verified.
  • ✓ Leave is shown for reference only and never double counted.
  • ✓ Move costs, healthcare and the cost-of-living adjustment are clearly separate lines, each from you.
  • ✓ Runs entirely in your browser. Your salary, offer and destination are never sent to a server. Share links hold your inputs in the link only, and the destination label is left out.

Frequently asked questions

Why does this tool not show what nurses or doctors earn in Australia, Canada or the US? +
Because no reliable source for that exists outside each country's own regulator or statistics office, and most comparison tables online are built on stale, currency-converted averages that can mislead a decision this big. Pay for the same role varies by state, city and employer. This calculator is built around that principle: every figure for the abroad side comes from you, from a real offer or from a primary source you have checked. The only numbers we supply are exchange rates, which are market data rather than pay data.
So what does the calculator actually do? +
It works out your real NHS take-home pay and one year of NHS pension using the same engines as our Pay and Pension calculators, then holds that against the figures you enter for an offer: net or gross pay, any employer retirement contribution, any healthcare cost you would carry, and the one-off cost of the move. It converts to pounds at an editable rate, shows both sides line by line, and tells you the net pay an offer would have to reach to match your NHS package.
What is the break-even figure? +
It is the net annual pay in the offer's currency at which the abroad package would exactly equal your NHS package, after your healthcare cost, any employer retirement contribution and your move costs spread over the years you expect to stay. If you also tick the cost-of-living box it shows a second figure that matches your cost-of-living adjustment too. It is useful because you can hold any offer up against one number instead of re-running the comparison.
Where do the exchange rates come from? +
They are mid-market GBP rates for 7 Oct 2026 from ExchangeRate-API's open data feed. The AED and SAR are pegged to the US dollar (3.6725 and 3.75 per dollar), so their rates move with GBP to USD. Rates move daily, so every rate field is editable and links to XE so you can check today's rate before relying on the result.
Why do you ask me for the effective tax rate instead of calculating it? +
Because we do not have, and will not fabricate, a foreign tax engine. Tax systems vary by country and often by state or province, and a confidently wrong figure is worse than none. If you only have a gross figure, enter an effective tax rate you have verified with the destination's own tax authority. If you can get a net figure from an offer letter, payslip or the country's official take-home calculator, use that instead.
Why does the NHS side count pension but not annual leave? +
Both sides are full-year salaried roles, so leave is already paid for inside the salary on both sides. Adding it would count it twice. Leave is shown for reference only. Pension is different: it is extra remuneration, so the NHS side counts one year of 2015 CARE pension (valued at 20 times the annual pension earned) and the abroad side counts any employer retirement contribution you enter.
What is the cost-of-living adjustment? +
An optional percentage that you supply after checking a cost-of-living comparison yourself, saying how much more or less expensive your destination is than where you live now. When you give one, the calculator shows a second, clearly separate figure alongside the nominal comparison. The two are never blended, because a cost-of-living percentage is a much softer input than a pay figure.
What happens to my NHS pension if I leave to work abroad? +
With at least two years' qualifying membership, the pension you have built is normally kept as a deferred pension and revalued until you draw it. With less than two years you are generally entitled to a refund of your own contributions instead. Transferring an NHS pension to an overseas scheme is harder than most people expect and is a common scam route. Our guides on deferred benefits and on QROPS cover the detail; confirm your own position with NHS Pensions before you leave.
What costs should I put in the move costs box? +
Visa and registration fees, assessment or exam fees, flights, shipping, deposits and a few weeks of temporary accommodation, all as quoted by the relevant regulator, immigration site or provider. Do not guess: fees change, so check the official source for your destination. The calculator spreads the total over the years you expect to stay and, if the offer pays more, tells you how many years of the extra pay it takes to recoup them.
Is this financial or career advice? +
No. It applies real NHS pay and pension mechanics to figures you provide so the comparison is like for like. Whether to leave the NHS for a role abroad depends on things it cannot see: demand for your specialty, registration and visa timelines, family, and your appetite for risk. Take real offers, primary-source tax and immigration guidance, and independent financial advice into the decision.
